Apollo 14, 1971, FLOWN Gold Foil. 5") mounted on a typed COA that reads: "This is to certify that the attached material was part of the outer reflective skin of the Apollo14 Spacecraft that carried astronauts Al Shepard, Ed Mitchell and Stu Roosa on their historic flight to the moon January 31 to 9 February 1971". It is signed by Terry Slezak - NASA MSC - Lunar Receiving Lab Decontamination Team. It was after Apollo 11 that Slezak achieved NASA immortality by becoming the first man to touch Moon Dust (inadvertently!). Rare.
