Partially Printed Document Accomplished in Manuscript, Two pages, recto/verso, Los Angeles, California, January 31, 1964. An application to AFTRA, signed and completed by Jay Silverheels, who portrayed the laconic side kick "Tonto" in the popular series, "The Lone Ranger." Never at a loss for words, the unflappable professional lists his occupation as "Actor." Signed twice in full at the bottom of the first page. Fine.
